Application Packet

The enrollment application packet contains several pieces of information required by state law, as well as several required by Mount Eagle Academy. Please download documents. All documents are in Adobe PDF format. We ask you to read and fill out all forms using the Registration Checklist as your guide to ensure your application packet is complete. Applications will be accepted until May 25, 2012 or until we are full, whichever comes first.

  3. A contract between Mount Eagle Academy private school satellite and your family setting forth our agreement. PSP Agreement
  4. Mount Eagle Academy is a Christian school and all our guidance will be Christ—centered. Please carefully read the Statement of Faith. Please do not sign the statement or join if you are not in full agreement with the Statement of Faith. Statement of Faith
  5. We ask that all family members cooperate with the behavior guidelines. We are ambassadors for Christ. Code of Conduct
  6. A listing of the qualification you and your spouse have to teach your children, the most important being you are the parents. Include work or volunteer experience, degrees, certificates or other education classes or seminars, meetings, training, or lectures you or your spouse have attended, or anything else you feel qualifies you as your child's teacher. Upon acceptance to Mount Eagle Academy, the following forms will need to be submitted:

  1. In addition, the State of California requires all private schools to have a copy of each student's birth certificate, immunication and health forms. If immunization or health exams are against your personal belief, please submit a waiver. Please submit a copy of your child's immunization record from the pediatrician's office.
